Home Care Patient and Family Advisory Council
A patient and family advisory council (PFAC) is an organization of current and former patients, family members and caregivers that works together to advance best practices. Learn more about the importance of a PFAC.
Our Home Care PFAC collaborates with home care leadership and staff to enhance the patient and family experience. The group meets monthly to discuss new programs, share educational materials and stay informed about our work.
If you are a current or former patient, family member or caregiver who is interested in helping to improve the delivery of safe and quality health care, consider joining our PFAC. For more information on how to join, contact our patient experience team at [email protected].
